Formula and thresholds
SD score = (measured length − mean for age) / SD. Micropenis: below mean − 2.5 SD.
Clinical note
Measurement: hold the glans and stretch maximally without pain; press a rigid ruler against the pubic bone and measure along the dorsal surface to the tip of the glans, excluding the foreskin. Not compressing the suprapubic fat pad underestimates length (buried penis). A result below the micropenis cutoff calls for assessment of the testes, hypospadias and endocrine evaluation (LH, FSH, testosterone, during mini-puberty). The norms come mainly from European and North American populations.
